Useful forms for ISHA members
This collection of sample forms is offered as a source of model forms for ISHA member organizations needing to create forms for various elements of museum or hall of fame activity. While they are offered as examples only, and should be reviewed by the appropriate department or professional consultant at your organization before being adapted for your use, these model forms may save you time and effort, as they are based on working forms used by ISHA member organizations.

Each form is listed within its appropriate category by its topic, with an indication of the source institution; at the end of this listing is a key to the sources for these model forms. In some cases, there is more than one example form for a given procedure.
